To make sure you qualify for certification, please answer the following questions and enter the required information.

If you have more than one qualifying medical condition, you only need to provide documentation for one condition. Once you qualify for the Card based on one condition, we can advise you on which cannabis products would be best to address each and all of your conditions. So, whichever of the conditions that you would be able to get documentation on the easiest or quickest, that is the one you should pursue.


A) Your medical records from the doctor treating you for your qualifying medical condition as long as they show the diagnosis and treatment plan

B) You can obtain a letter from the doctor verifying the diagnosis of your qualifying medical condition and treatment plan.

C) Medical records can be obtained from a patient portal and used to verify your condition if you cannot get your records or a letter from your doctor. It should show your treatment plan and it would be helpful to include any relevant diagnostic tests such as MRIs, bloodwork, etc.

D) For a diagnosis of Intractable Pain, the records or letter from your doctor should verify the cause or source, the location, the severity, the frequency, and that you have been dealing with it for a while.
